Personalised Support Plans

Meeting with you to truly listen and understand what support you are looking for.

The support that you may need or want is specific to you, we are dedicated to designing the support package that you want and ask for to live your life.  We will meet with you at your choice of location, wherever you are comfortable, this can be in person or digitally.

 

The biggest part of meeting with you is to listen to you so we have a full understanding of what you are looking for.  You are the expert of your life and what you need and want to live your best life.  

We take the information from that meeting and create a support plan, this is a guide for your support crew to follow. It has your likes and dislikes, goals, activities you do, your weekly schedule and the support you require.  It also has important health and safety information.  There is also a separate page to explain the disability/ies, health conditions you have and how they affect your daily life.  All given to you to approve before it is shared with any support crew.

 

A copy of your support plan is with you at home and the plan is shared digitally with each of your support crew.  This plan can be adapted whenever you want.
